Molekularpathologische Diagnostik

In Zusammenarbeit mit dem Institut für Allgemeine Pathologie können wir Ihnen ein breites Spektrum molekularbiologischer Untersuchungsmethoden anbieten, die zum einen zu einer verfeinerten Hirntumordiagnostik beitragen und zum anderen zusätzliche klinisch relevante Informationen zur individuellen Prognose und dem Ansprechen der betroffenen Patienten auf bestimmte Therapieformen liefern.

Im folgenden werden die von uns angebotenen relevanten molekularpathologischen Untersuchungsmethoden kurz aufgeführt. Detaillierte Informationen entnehmen Sie bitte auch einer aktuellen Reviewarbeit (Riemenschneider et al.; Acta Neuropathol. 2013; 126: 21-37).

MGMT-Methylierungsanalyse (quantitativ):

Aussagekraft: Prädiktiv für das Ansprechen von älteren Gliompatienten (>65 Jahre) auf eine alkylierende Chemotherapie. Prognostisch in anaplastischen Gliompatienten, die mit einer Radio-und/oder alkylierenden Chemotherapie behandelt werden.

Methodik: Die Methylierungsanalyse erfolgt mittels der Methode der Pyrosequenzierung des MGMT-Promotors an Formalin-fixiertem und Paraffin-eingebettetem Tumorgewebe. Für den MGMT-Test bedarf es daher der Einsendung eines Paraffinblöckchens mit repräsentativen, vitalen Tumorgewebsanteilen (mindestens 60%).

Untersuchung auf Allelverluste der chromosomalen Regionen 1p und 19q in oligodendroglialen Tumoren:

Aussagekraft: Prädiktiver Marker in anaplastischen oligodendroglialen Tumoren für das Ansprechen auf eine PCV-Chemotherapie. Prognostisch günstiger Marker in (oligodendro-) glialen Tumorpatienten, die mit einer adjuvanten Radio-/Chemo-therapie behandelt werden.

Methodik: Der Nachweis der deletierten chromosomalen Regionen erfolgt mittels chromogener in situ-Hybridisierung (CISH) und/oder Mikrosatellitenanalyse (LOH) an Formalin-fixiertem und Paraffin-eingebettetem Tumorgewebe. Für die LOH Untersuchung ist die zusätzliche Übersendung von Patientenblut (5ml EDTA-Blut) erforderlich.

Mutationen im Codon 132 des IDH1 und im Codon 172 des IDH2-Gens:

Aussagekraft: Diagnostischer Marker für diffuse Gliome der WHO-Grade II und III sowie für Patienten mit sekundären Glioblastomen und in diesen Patienten mit einer günstigeren Prognose assoziiert. Selten in primären Glioblastomen, jedoch -wenn dort nachweisbar- ebenfalls mit einer besseren Prognose assoziiert.

Methodik: Neben einem Mutations-spezifischen Antikörper für die häufigste Mutation im IDH1 Gen (R132H), erfolgt der Mutationsnachweis unter Anwendung eines Next Generation Sequencing (NGS) Panels an Formalin-fixiertem und Paraffin-eingebettetem Tumorgewebe.

V600E-Mutationsanalyse:

Aussagekraft: Diagnostischer Marker für pilozytische Astrozytome, pleomorphe Xanthoastrozytome und Gangliogliome. Hilfreich für die Unterscheidung dieser Tumoren von diffusen Astrozytomen (WHO-Grad II).

Methodik: Der Mutationsnachweis erfolgt unter Anwendung eines Next Generation Sequencing (NGS) Panels an Formalin-fixiertem und Paraffin-eingebettetem Tumorgewebe.
